What Counts as a Medication Error?

A medication error occurs any time a resident receives the wrong drug, the wrong dose, medication at the wrong time, or no medication at all when it was ordered.

Medication errors may involve:

• Administering the wrong drug
• Giving an incorrect dosage
• Missing or skipping scheduled doses
• Administering medication at the wrong time
• Failing to check for drug interactions
• Giving medication to the wrong resident
• Ignoring a known allergy

Some errors cause sudden, obvious harm. Others cause a slow decline that's harder to trace back to its source.

Why Medication Errors Happen in Nursing Homes

These mistakes are rarely about one person having a bad day. Common contributing factors include:

Understaffing

A single nurse responsible for far more residents than is safe is far more likely to rush, skip steps, or miss warning signs.

Poor Communication During Handoffs

Shift changes are a common point of failure, when one nurse's knowledge about a resident's medications doesn't fully transfer to the next.

Inadequate Training

Staff without proper training on dosing, interactions, or administration procedures are more likely to make preventable mistakes.

Outdated or Disorganized Records

When pharmacy records, physician orders, and administration logs don't match, errors become far more likely, and far harder to catch.

Research reviewing medication error studies in long-term care found that between 16 and 27 percent of residents were affected by some type of medication error, and some estimates put the scale of preventable medication-related injuries in U.S. long-term care facilities at roughly 800,000 incidents per year.
